Summary:PT Laksana Karis Industri (LKI) is a metal processing company that produces electrical support equipment and telecommunications networks. PT LKI experience an increase in the percentage of defective products when production levels are higher. Defective products that cannot be repaired cannot be resold, so it becomes a waste and causes losses for the company. Therefore, the company need to reduce the number of defective products by improving the quality of the products produced by the company. Product quality can be improved by reducing variations that cause defects in the product. One of the methods used to identify and describe the causes of defects is the Define, Measure, Analyze, Improve, and Control (DMAIC) method. At the Define stage, the type of defect in the product is determined. At the measure stage, the value of stability and production capability is calculated using a control chart. In the analysis phase, the factors that cause defects in the product is define by using the Delphi method. In the improve stage, a proposed improvement is designed for each root cause of the problem. In the control stage, a schedule for the implementation of each proposed alternative solution is formulated. Based on the research results, five types of defects have been identified. The types of defects in BMF products are defects in the hole that does not fit and the position of the hole that is tilted, while the types of defects in the V Bolt product are thread breaks, broken bends, and improper bend positions. The research is focused on five main root cause problems, namely operator fatigue, machine components that suddenly wear out during production, poor raw material quality, blunt cutting edges, and non-standard rotational speed. The design of proposed alternative solutions is expected to be able to improve product quality so reduce the number of defective products.
