Kartu Obat is a system that have been developed by Dr. Hasan Sadikin Hospital Bandung to replace the prescription system. This system has some modification from the prescription system for the purpose of ensuring Obat Satu Pintu System, ensuring drug’s quality, and ensuring drug’s use record. Research for Kartu Obat system is need to be conducted to know how much this system could replace the prescription system. Besides that, the objective of this research is to know the weakness of the system and to find the solution. The research started with with field overview, comparison of Kartu Obat and prescription, observation to the pharmacy satelite that uses the Kartu Obat system, interview, data analysis, conclusion. The result showed that Kartu Obat has 6 out of 8 main components of the prescription by the literature, they are prescriber office information, patient information, date, medication prescribed, signatura, prescriber’s signature. There are weakness in the system that results in 17,65% case of misuses of administration type and 63,24% case of incomplete fill of Kartu Obat. Part of the Kartu Obat that most often blank are diagnose (57,35%) and ward/bed information (51,47%). There are 2 different numbering formats (Arabics and Romans) found in the usage that could cause misintrepretation of Kartu Obat. From the interview’s analysis results, there are 71,43% patient’s family member in Ward 17 which claim that Kartu Obat was not affecting drug’s taking progress. Meanwhile, there are 89,47% nurse in Ward 17 claim that Kartu Obat help their task and 84,21% reason from the number claim that Kartu Obat help therapy monitoring. The conclusion showed that Kartu Obat has the form to replace the main function of the prescription, but there are some weaknesses that could allow the medical personel to break the rule of the system.
