inappropriate or harmful use of medication for a patient while under the supervision of healthcare
professional or the patient himself. Medication errors can occur in every medication process, one of which
is during the prescribing step. In August-September 2022 there was an increase in cases of acute kidney
failure in children (Gangguan Ginjal Akut pada Anak—GGAPA), the cause of these events is not certain and
the use of medicinal syrup for children is temporarily prohibited. The temporary ban on the use of
medicines in syrup dosage forms has led to an increase in the demand for extemporaneous drugs for
children. The surge in demand for these extemporaneous drugs has made the role of pharmacists and
other health professionals important in preventing medication errors from occurring. The purpose of this
study was to identify and determine the incidence of medication errors at the prescribing stage and to
determine the correlation between the increase in the number of prescriptions for extemporaneous for
pediatric patients and the incidence of medication errors at the UPTD Puskesmas Garuda. The research
method used in this research is descriptive observational with a cross-sectional design. The subjects of
this study were prescription sheets for pediatric patients at the UPTD Puskesmas Garuda on September
15th to November 15th 2022. Data collection in this study was carried out retrospectively using a purposive
sampling technique. The results of the study found that the incidence of medication errors at the
prescribing stage in prescription concoctions in pediatric patients at the UPTD Puskesmas Garuda Bandung
City was 954 errors from 320 prescription sheets. Based on the results of the study, there were errors in
writing the name of the drug (0,10%), incomplete prescriptions (96,96%), illegible prescriptions (2,94%),
and no errors in the use of drug abbreviations. The results of the correlation test using the chi-square
method showed that there was no significant effect (P-value > 0.05) between the increase in the number
of prescriptions for concoctions for pediatric patients and the incidence of medication errors at the UPTD
Puskesmas Garuda, Bandung City. Based on the results of the study using the chi-square correlation test,
there is no significant relationship between the increase in the number of prescriptions for concocted
drugs for pediatric patients and the incidence of medication errors at UPTD Puskesmas Garuda Bandung
City.
