The increase of asthma attack is triggered by the environment, poor adherence of health personnel towards guideline therapy, non patient adherence to asthma medication, as well as improper use of inhalers. The objective of this study is to examine the use of corticosteroids in asthma medication based on literature review and guideline therapy including indications, dosage, drug interactions, and patient compliance. The study was conducted by observating retrospectively through searching for prescriptions and medical records for the period October - December 2009 and also by distributing questionnaire in the outpatient with asthma in pharmacy of Askes to assess the compliance. Out of 239 prescriptions, 62% received glucocorticoid inhalation, 31% received glucocorticoid oral, and 7% received combination of both. The dosage ranges for oral glucocorticoids were from 20 to 60 mg and for inhalation dosage were 200 to 1200 µg. The dosage ranges were in accordance with the literature and guideline therapy. Corticosteroids, especially given by oral rute, interacted with theophylline, erythromycin, and salbutamol, but is not significant clinically. Assessment of compliance obtained from the questionnaire stated that 73% of 15 patients with asthma in the pharmacy of Askes were adherent to treatment. However, 36% of them still often suffered from exacerbation although they used inhaler appropriately, 18% of them still often suffered from exacerbation and they used inhaler inappropriately, and 46% of them have not suffered from exacerbation although they used inhaler inappropriately. There were 27% nonadherence asthma patients and they used inhaler inappropriate too. text
