Summary:The traffic police recorded truck vehicles as the second largest contributor to traffic accidents in 2020 and 2021 respectively with a large number of losses, both materially and fatalities. However, there is no literacy source that provides comprehensive information related to the causes of traffic incidents involving trucks from various types of potential factors. This study aims to identify these causal factors thoroughly through the design of a questionnaire instrument. The questionnaire design method used in this study follows the stages of the Macroergonomic Organizational Questionnaire Survey (MOQS). The focus of the questionnaire model is only to associate various indicators of several causal factors with the respondent's history of involvement in accidents and near-misses. The respondents consisted of 21 drivers who came from different trucking companies. The results showed that the factors causing accidents and near-miss were most influenced (64% and 39%) by fatigue or drowsiness. In addition, other factors such as the environment, vehicles, traffic, and companies, respectively, also contribute to several traffic incidents. Then, it was also found that there was a significant influence between the level of education, alcohol consumption habits, and the driver's experience with accidents and near-misses. Through the discovery of comprehensive causal factors, the authorities are expected to design targeted solutions in order to improve driving safety systems, especially truck transportation in Indonesia. For future research, it is expected to collect more number of respondents and evaluate the contents of the questionnaire to dig deeper related to the information to be obtained.
