Summary:PT Cahaya Perada Abadi (PT CPA) is a manufacturing company in the textile industry, primarily producing golf gloves. This study focuses on the Q3 type golf gloves, which constituted approximately 68% of the total glove production in 2023. The production process of golf gloves includes stages such as cutting, preparation, subassembly, assembly, finishing, and ends with final quality control. At PT CPA, there is no historical data on the mapping of occupational health and safety (OHS) risks, even though this information is crucial for the company to consider suitable work activities and environments. Initial interviews also revealed frequent workplace accidents, such as needle pricks during sewing and body aches due to non-ergonomic working conditions. Therefore, the company has stated the need for an evaluation of risk levels at PT CPA to reduce potential hazards. This study evaluated hazard risk levels using the HIRADC method and identified 19 sub- activities with major risk, 45 with medium risk, and 29 with low risk. However, many of the proposed risk mitigations involve administrative control and the use of personal protective equipment, which are deemed inefficient, necessitating further evaluation. Thus, this study also identified error traps by adapting the WTTT method. From the error trap mapping, risk mitigation in the form of engineering control was proposed by designing prototypes for three activities with the highest severity levels. The completed designs were then shown to workers for feedback. The designs include a sewing chair and sewing machine modifications to address error traps in sewing activities, and a container design for reinforcement stickers to address error traps in sticker application activities. This risk mitigation design is expected to reduce quality risks and OHS risks in the company.
