Summary:The Indonesian National Police recorded that trucks were the second-largest contributor to traffic accidents in Indonesia after motorcycles in 2022, causing numerous fatalities and significant financial losses. However, there has been limited research surveying the factors related to truck safety issues, particularly concerning truck accidents. Therefore, this study aims to develop a questionnaire for the initial identification of truck safety issues on highways, considering the operational characteristics prevalent in Indonesia. The questionnaire development process was conducted following the stages of designing the Macroergonomics Organizational Questionnaire Survey (MOQS). This process began with conceptualization, designing a conceptual framework for the questionnaire based on a reference framework, compiling questionnaire items based on a literature review, designing the questionnaire layout, and concluding with preliminary testing of the questionnaire. Forty respondents from various companies participated in this study. The result of this research is a successfully developed questionnaire consisting of 75 questions, including 50 closed-ended questions, 24 closed-ended questions with Likert scale answers that were validated and deemed reliable with a Cronbach’s alpha value of 0.874, and one open-ended question. Therefore, the results obtained from this questionnaire can be processed and analyzed further to obtain an initial overview of truck safety issues in Indonesia. This questionnaire can be used as a reference for subsequent research to identify factors related to truck safety, particularly truck accidents in Indonesia, more comprehensively with a larger sample and accommodating more types of trucks.
