This research specifically evaluates the causal factors of workplace accidents at the Pakuwon Residences construction project, PT Grama Pramesi Siddhi (GPS). A hybrid FMEA method combining FMEA, FTA, and ETA was employed. The FMEA analysis revealed that the accidents with the highest risk priority number (RPN) were falls into foundation pits, being struck by debris from the 22nd floor, finger entrapment in a bar bending machine, nail punctures, and being struck by bentonite bore pile. A qualitative FTA of the five priority accidents with the highest RPN indicated that 73% of basic events were unsafe conditions. ETA analysis demonstrated that if corrective actions were taken for each major initiating event, the potential for accidents could be significantly reduced. Multiple regression analysis of self-assessment questionnaires based on ISO 45001 distributed to management showed that the implementation of an occupational health and safety management system had a 0.19 impact point (Regression Coefficient: -0.19) on reducing accidents, while the implementation of OSHA for construction distributed to workers had a 0.062 impact point (Regression Coefficient: -0.062) on reducing workplace accidents when implemented. This indicates that both systems may not be optimal in preventing workplace accidents. text
