Background: The resulting digital panoramic radiography is determined from radiographs making process itself. Technical error and position error are the common reasons behind the failure of creating panoramic digital radiograph. Purpose: To research the failure of creating panoramic digital radiograph at RSGM Faculty of Dental Medicine Airlangga University (period October-December 2014). Methods: 100 Samples of failure that match the criteria are examined and tabulated based on the characteristic that appeared. Result: 78 of 100 samples had position errors dominated by failure to position the tongue against the palate, whereas 2 had technical errors Conclusion: The position error is the most common mistake in digital panoramic radiography in UPF Radiology RSGM Faculty of Dental Medicine Airlangga University (period October-December 2014.
