Introduction: In the elderly there is a process of change in the form of deterioration such as the number of muscles decreases, the heart muscles become degenerative (problems with the cardiovascular system), the blood vessels become stiff, the bones become porous, memory decreases, vision and hearing begin to decrease. The heart is a clinical syndrome which is characterized by dyspnea and shortness of breath caused by several forms of heart function disorders. The research aims to explain medical-surgical nursing care for the elderly who receive gerontic care at the UPTD Panti Nursing Home Jambangan Surabaya, including arranging bed positions and doing deep breathing exercises. The aim of this study was to apply the semi-Fowler position and deep breathing exercises to elderly people with ineffective breathing patterns in patients with congestive heart failure. Method: this research is a case study with a single case. The research respondents were elderly people with ineffective breathing patterns. The instrument used is the Carol A.Miller model of elderly nursing care format which includes assessment, diagnosis, intervention, implementation and evaluation. Results: research found that the client was 80 years old with ineffective breathing patterns, shortness of breath, and tired easily. The nursing diagnosis raised was ineffective breathing pattern. The intervention was carried out in accordance with SIKI with semi-Fowler position therapy and deep breathing exercises. Discussion: After implementing 3x24 hours, all nursing diagnoses have been resolved, breathing patterns have improved, signs of dyspnea have decreased and respiratory frequency has improved. The provision of semi-Fowler position therapy and deep breathing exercises needs to be carried out continuously with the supervision of accompanying staff.
