Summary:The hemoglobin level test using cyanmethemoglobin method is influenced by leukocytosis which significantly increased absorbence measurements of hemoglobin levels inaccurate that makes diluted blood samples with Drabkins reagent by centrifugation at 3000 rpm for 10 minutes, in order to measured supernatant absorbence with photometric at λ 546 nm. The purpose of this research is to analyze the differences of hemoglobin levels test using cyanmethemoglobin method with and without centrifugation on leukocytosis sample. Type of this reseach is observational laboratoric with cross sectional study design.Tested blood samples were taken from the remaining blood samples of patients who had leukocytes measured more than 20.000/μL with Hematology Analyzer (CELL-DYN Ruby), divided into 4 groups based on a number of leukocytes criteria. Group 1. 20.000/μL-<30.000/μL, group II. 30.000/μL-<40.000/μL, group III. 40.000/μL-<50.000/μL, group IV. ≥50.000/μL. The number of samples taken is 20 samples per group. Total number of sampel is 80 samples. The result of analysis showed that there is no significant difference in of hemoglobin levels using cyanmethemoglobin method with and without centrifugation leukocytosis sample with value of p = 0,000 is smaller than 0,05. Turbidity caused by leukocytosis effected the difference of hemoglobin levels with and without centrifugation, as tested, the higher leukocyte number the greater differences of leukocyte hemoglobin levels. More on test result of grouped criteria, shows differences leukocyte counts obtained by the difference in hemoglobin levels with and without centrifugation in group I. 0,22 ± 0,07 g/dL, in group II. 0,40 ± 0,22 g/dL, in group III 0,44 ± 0,14 g/dL, in group IV. 0,85 ± 0,41 g/dL. The hemoglobin level test using cyanmethemoglobin method on leukocytosis samples with more than 20.000/μL is necessary to centrifuge to obtain suitable hemoglobin levels with the patient's clinical condition.
