The work accident remains a big problem of occupational health and safety in Indonesia that proved by 24.910 work accident which were occured in 2014. Work accident isn’t a problem that can’t be prevented and solved.. Oil and gas company, as PT X, is an industry with high potential of hazard so that work accident can occur easily, as good as fatality or minor accident. This study aims to analyze work accident that was recorded within 2013 to 2015 in PT X. While the further analyze was to study many possible factors that influence the work accident by using Bird (1967) theory about International Loss Control Institute(ILCI). This is a descriptive study which using primary an secondary data. The primary data were collected by means interviewing the victims, who still working in PT X when the study is held, interviewing senior superivisor process safety management, and doing some observation, then secondary data were obtained by work accident statistic data which collected from Januari 2013 to Desember 2015. The result of this study showed that the number of accident seems like going to be higher than the same study before in 1994-1996. Based on the result shows that the incident rate was about 0,81% in 2013-2015, the frequency rate was 3 times in every 1.000.000 manhours, and the severity rate was approximately 347 lost workdays in every 1.000.000 manhours caused by work accident. Based on work accident charateristics, kind of work accident were mostly splashed by corrosive materials (23,32%), mostly occured in RCC unit (18,33%), part of body which mostly being effected by work accident were hand (40%), most occured in the beginning of first shift (28,33%), and minor accidents were mostly occured (65%). The results will bring into a conclusion that location of work accident were perhaps the most influenced factor in this study. Not only that, but also lack of management, in this case is lack of commitment to implement occupational health and safety system, is the main causes of accident could be occured.
