Summary: | background: external root resorption (ERR) that present below the alveolar crest makes the treatment diffcult. if ERR associated with extensive furcation perforation, the treatment becomes even more challenging. purpose: the aim of this case report is to descibe the management of an ERR and extensive furcation perforation using mineral trioxide aggregate (MTA) and root hemisection. Case: A 53 year old male patient with nanocontributory medical history complained food inpaction in the lower right first molar. clinical examination showed grossly decayed lower right first molar with large amalgam restoration and crown fracture in the mesial of the tooth. there is no subjective pain neither tenderness to percussion or palpation. the radiographic examination revealed a non-hermetic post endodontic with irregular radiolucency in the cervical third of the mesial root along with the furcation. the distal root presented with an external resorptive defect at the jungction of cervical. case management: retreatment was completed in the distal root only. as the furcation perforation was too extensive on the masial side, hemisection of the mesial root was performed . the external resorptive defect was then curetted and repaired with MTA. after 1 month of follow-up, the tooth was restored with porcelain fused to metal splint crown. conclusion: ERR associated with extensive furcation perforation could be managed with MTA and root hemisection which prolongs the survival of the remaining tooth structure and providing favorable prognosis.
