Summary:Objectives. The aims of the study were to develop pharmaceutical care practice model, then to evaluate the effect of the model in patients’ knowledge, attitudes and practice toward the use of their diabetic medicine. Methods. A semi-structured interview to 20 pharmacists and 20 diabetic patients were done as phase 1 of the study. The results of the phase 1 and literature review were used to develop the model. A focus group discussion was done to improve the model. Then a quasi experimental, non equivalent control group pretest postest design was conducted for 4 months. Pre test measurement was done before the intervention, post test 1 was done in the first month after the intervention, and post test 2 was done in the third month after the intervention. There were 16 pharmacists in the control group and 16 pharmacists in the intervention group, each from a different pharmacy, with 4 patients taken from each pharmacy. Results. Pharmaceutical care practice model consist of order sequence of pharmaceutical care practice, arranged in a module, provided with training, and equipped with practical tools for practice. There was an increased knowledge and practice of pharmacists as a result of the application of pharmaceutical care practice model. While the pharmacist attitudes after the application of pharmaceutical care models were not improved. Pharmaceutical care practice model intervention in the community pharmacy showed a significant change in the knowledge, attitude and practice of diabetic patients. Conclusion. To conclude, pharmaceutical care practice model showed benefit in improving patient knowledge, attitudes and practices toward medicine.
