In Indonesia,stroke is the leading cause of death either in urban ruraL area. Stroke death and disability are related to arrival time al hospital. This study aims to describe stroke case by demographic characteristics, warning sign and to analyza factor related to arrival time at hospital. The survey was conducted in Surabaya, The second biggest city in Indonesia. samples ware 111 stroke cases of Surabaya inhabitants arriving in fours hospital in Surabaya which mare selected randomly. Descriptive analysis of the variable and a chi square test were used to perform association between the demographic characteristic and warning signs as independent variables, and arrival time at hospital as dependent variable. Stroke affected people age 30 years and older whereas most of them (41.4%) were 50-59 years old. Almost Balf of stoke case (45.5%) bad insurance for poor people and 90% of them lived with their family. The Most frequent case of risk factors (6.3%)was hypertension. futhermore,half or less stroke case knew that they bad either diabetes mellitus or hyperbolestrolemia. Most warning sign of stroke case were weakness on one side of the body (80.2%) and sudden lock of walking coordination (37.8%). Less than third (27%) of stroke case arrived in Hospital in hours or less after stroke attack and factor related to it was mate. Then, the warning sign significantly associated with the arrival time at hospital was pomiting (p value <0.05). Epidemiologic transiting is faced in Surabaya. Stroke affected people not only in high level of social economy but also poor people and must of stroke cases arrived in hospital more than 3 hours after stroke attack
