Background: Infertility score is the instrument use to facilitate referral system in order to take the infertile treatment based on the level of health facilities (primary, secoundary, and tertier). IVF the most recent measure taken by infertile couples in an attempt to get pregnant, available in endokrinology fertility and reproduction with the assisted reproduction technology in tertier health facilities. In fertility clinic of Graha Amerta Goverment Hospital Dr. Soetomo Surabaya in the last three years, IVF has increased almost twofold from 2014 until 2016 coming from referral and non-referral cases. The purpose of this research was to analyze differences in referral and non-referral case on IVF measures. Method: This study was descriptive observasional with cross sectional design using secondary data. A total sampling technique was used, found 342 of referral and non referral infertility cases. Dependent variable is infertility score and independent is referral and non-referral cases. Results: From 342 cases, there was 44,2% wives’ age is in the rank of 30-35 years old, while the husband’s age was 48% in the rank of > 35 years old. Primary infertility type was found in most of the referral case of 78,7% and the non-referral case of 73,6%. Infertile in referral and non-referral cases was found mostly due to wives’ factor (68,1%). Severe infertile score (≥12) was found in almost entirely found in referral cases (92,2%), while mild infertility score (≤8) was found in the non-referral cases of 16,4%. The results of statistical tests conducted using Chi Square with α <0.05 obtained p value = 0.001. Conclusion: There is difference of infertility score in referral and non-referral case on IVF.
