Summary:Background: Fast health service development which is not followed by economic growth of the community limits access of the community to convenient, affordable and standard health service. Muhammadiyah Health fund is an organization established by the management of Muhammadiyah at Yogyakarta territory to plan, manage and pay health cost for its members. Health service of Muhammadiyah Health Fund is provided in every institution signed memorandum of understanding. Presently there are 34, 474 participants of Muhammadiyah Health Fund with as many as 35 paramedics, consisting of 18 general practitioners, 11 dentists and 6 paramedics. Health service at the primary level is provided at the polyclinic and at the secondary level is at PKU Muhammadiyah Hospital Yogyakarta. So far there is no control applied to primary health service to limit over utilization. A study on the utilization and cost of secondary level service has never been carried out so that increase in the utilization and cost is going on. Objective: To identify the utilization pattern of health service for Muhammadiyah Health Fund participants at PKU Muhammadiyah Hospital Yogyakarta and find out the cost most dominantly used in secondary level service. Method: This descriptive study used a case study design. Subject of the study were all participants of Muhammadiyah Health Fund. Data were obtained through medical records at Muhammadiyah Health Fund of Muhammadiyah Yogyakarta University and PKU Muhammadiyah Hospital. Data were presented in narration and tabulation and the result of analysis was described quantitatively to identify the utilization of service, number of visits, rate of visits, cost of outpatient and inpatient of Yogyakarta Muhammadiyah Health Fund participants. Result: The total cost of outpatient and inpatient within August 2006 � 2007 was Rp 404,221,996
