Summary:Epilepsy is one of the important public health issue and prominent in the community, because the problem is not only medical but also in terms of social and economic development affecting patients and their families. In Indonesia, the prevalence of epilepsy is estimated from 0.5 to 1.2%. Antiepileptic drugs have a dominant role in the management of epilepsy. Although monotherapy is the main option, polytherapy still difficult to avoid. This polypharmacy is unsatisfactory because of these reasons: (a) increase the risk of chronic toxicity, (b) by monitoring the drug levels in the blood, an antiepileptic drug can be administered more effectively, (c) the lack of evidence regarding the effectiveness of two (or more) different drugs compared with the using of one drug only. This study aimed is to know the efficiency and safety toward the monotherapy treatment compared with polytherapy by analyzing the seizure frequency, seizure-free state and adverse drug events. The data analyzed by using the seizure diaries and APS (Adverse Drug Reaction Probability Scale). This study used observational study design with prospective data collection, comparing the two groups of subjects suffering from idiopathic epilepsy general tonic clonic type with onset in adulthood. The first group is a monotherapy, the second group is a polytherapy. The effectiveness of therapy was measured by looking at the frequency of seizures, duration of seizure-free and drug side effects experienced by patients. The inclusion criteria included are as follow: (1) the type of seizure is idiopathic general tonic clonic, (2) get treatment at the clinic of Dr Sardjito in the period May-June 2012 (3) the using of the drug regularly. Therapeutic efficacy in seizures controls were analyzed by chi square test. Monotherapy group had a large proportion in the control of seizures than in polytherapy views of seizure frequency with a significant difference (p <0.05). Besides that, it can be seen from the number of seizure-free patients, monotherapy treatment is better in controlling seizures. These results can be seen from the number of subjects who experienced seizure-free on monotherapy: there are 29 patients of monotherapy (83%) compared with polytherapy patients is 17 (48%). The prevalence of side effects of the drug in monotherapy compared with polytherapy was 20% v.s 14%. A although monotherapy give adverse drug events greater than polytheraphy, the interviews shows that the patients on monotherapy experienced lighter and did not cause uncomfortability side effects than polytherapy.
