Summary: | Cirrhosis of the liver is frequently accompanied by complex alterations in the
hemostatic system, resulting in a bleeding tendency. Data from the U.S. demonstrate
that 50% of patients with liver cirrhosis will develop portal hypertension and
esophageal varices. The prevalence of esophageal varices in patients with liver
cirrhosis is about 50% - 80%, while deaths due to bleeding is 17-57%. Study in
Indonesia demonstrate the prevalence of rupture of esophageal varices in cirrhotic
patients by 70% with a mortality of 26.6%.

. There was indirect evidence that there
may be endothelial perturbation in cirrhosis, which might explain some clinical and
biological changes occuring in clinical setting
2
. The aim of this study was to evaluate
whether there is endothelial perturbation and to detect any correlation with platelet
count in cirrhotic patient with variceal bleeding.
A cross sectional study design was conducted in Dr. Sardjito Hospital in
october 2010- october 2011. von Willebrand Factor (vWF) and platelet plasma
measured from cirrhosis patient with history of upper gastrointestinal bleeding. Data
distribution was tested using Kolmogorov Smirnov statistical test. vWF plasma from
cirrhosis group and non cirrhosis was compare using independent t test. The relation
of plasma vWF and platelet count in cirrhotic patient was investigated.
Plasma vWF in patient with cirrhosis were higher than that of healthy group
(220.56 + 19.83) vs 92.85 + 18.19 ( p= 0.00). There was decrement platelet count in
pastien cirrhosis 101.21 + 41.82 x10
