Description
Summary:Introduction. Nonalcoholic Fatty Liver Disease (NAFLD) is the most common complication of obesity, in a non-alcoholic subject. Ultrasound is the most common method of imaging to diagnose and to evaluate hepatic steatosis. Doppler ultrasound is important in the evaluation of hepatic vasculature and parenchym. The changes of the hepatic parenchym may alter hepatic vein compliance, showed by abnormal Doppler waveform, bifasic or monophasic. Objective. To find the difference of hepatic vein waveform between the overweight and normal weight subjects, with or without NAFLD. This study also shows the important antropometric indicators to predict NAFLD. Methods. This is an analitic, cross-sectional designed study. The two groups of subjects, overweight and normal weight, with or without NAFLD, were taken by consecutive sampling non-random. Height, weight, waist circumference and hip circumference were measured, followed by liver grey-scale ultrasound and Doppler ultrasound of the right hepatic vein. The hepatic veins waveform is cathegorized into normal (triphasic) and abnormal (biphasic or monophasic). The data were analysed by comparative analysis (Chi-square). The p value < 0,05 represents the significant difference of hepatic waveform between overweight and normal weight, with or without NAFLD. Multivariate analysis were used for the correlation of variables. The other datas were analysed descriptively. Results and Discussion. Abnormal hepatics vein waveform is significantly higher in overweight group compared with the normal weight group, with or without NAFLD (p=0,001). But, the multivariate analysis showed that BMI itself did not have independent effect to the hepatic vein spectral waveform (p=0,158) compared with NAFLD, WHR, HC and WC. Waist circumference (WC), HC and WHR are the more superior predictors for NAFLD than BMI. Waist to Hip Ratio (WHR) 0,9 for man and 0,8 for woman, and WC 90 cm for man and 80 cm for woman represent the abdominal obesity, and can be used as predictors for NAFLD. Only WC 90 cm for man and 80 cm for woman have statistically significant correlation to the hepatic vein spectral waveform (p=0,001).
