Based on preliminary studies in RSJD Dr. RM SoedjarwadiKlaten still occur late return of medical record files of patients hospitalized in the general ward Mandiri I, incompleteness factor charging medical record file is the main factor that led to the medical record file late returns. Therefore, it is necessary to identify the factors causing delay in returning the inpatient medical record file public patients inRSJD Dr. RM SoedjarwadiKlaten. This study aims to determine the factors causing the delay in returning the inpatient medical record file public patients and the nation'sMandiri I know the procedure for the implementation of the return of inpatient medical record file in RSJD Dr. RM SoedjarwadiKlaten. This research is a descriptive study with a qualitative approach to data collection in cross sectional. Techniques of datacollection by interview, observation,andstudy documentation. Return of delay medical record file is still happening because of a lack of human resources, facilities and time. Means of support and the time to file medical record documentation is still lacking due to the high workload of physicians, thus charging less complete medical record file and cause delay in returning the inpatient medical record file on the ward Mandiri I as much as 75%.
