In 2004, Grhasia Yogyakarta hospital has applied Medicine Management Information System in Hospital Pharmacy Installation. The problems that still exist now are: the persistence of vacancies, there is still medication stockin the planning process is not appropriate, there is still a difference between the data manually and Medicine Management Information Systems, and frequent computer damage from lightning strikes, so the information system that are connected to the LAN to be broken. Find out it is good or bad quality of the output of a system, running or not existing applications it would require evaluation. Objectives: To evaluate the use of Medicine Management Information System in the management of medication in Hospital Pharmacy Grhasia Yogyakarta include components Input, Process, and Output. Methods: This research is a qualitative research, using a case study design. The data collected is a qualitative data. Information obtained from observation checklist format, review the documentation and in depth interview. Results: Use of medicine Management Information System in Hospital Pharmacy Installation Grhasia Yogyakarta there are two (2) kinds of models in the medicines management: billing system and inventory system. In terms of the process development of medicines management information systems do not involve the management. The lack of cooperation between the units, and there is no coordination well between the lines and inter related, between management and IFRS. The absence of SOP as a clear work guidelines and limits of work for officers who use billing system. Use billing system in Hospital Pharmacy Installation Grhasia Yogyakarta, all information has met the characteristics of good information that is accurate, timely, complete, relevant, reliable, verified, easy to understand, and easy to obtain. However, the use of inventory system the information generated generally has not fulfilled the characteristics of good information. Conclusions: The implementation of Medicine Management Information System in IFRS Grhasia Yogyakarta is not optimal due to the implementation of inventory system not use computer-based yet. So that the resulting output cannot use for decision making.
