Summary:Background: Health risk perception is a crucial component of most health behavior theories. Recent epidemiologic studies demonstrate that following a healthy lifestyle has substantial health benefits. It was reported that patient with negative perception on healthy life style have more tendency to be hospitalized and received admission for treatment. Objective: To examine the health risk perception and intention to adopt healthy life syle among primary care patients Methods: A cross-sectional study was used to assess the correlation between health risk perception and intention to adopt healthy life style in Yogyakarta, Indonesia.a number of 200 patients was undertaken from 10 health center for this study. Results: Healthy life style characteristics were as follows: unhealthy diet (70%), physical inactivity (93%) and tobacco smoking (16.5%). A total of 26 respondents (13%) perceived that smoking affect the illness, 49.5% perceived physical inactivity affect the disease, and 52.5% have positive perceptions to healthy diet. There were difference in the proportion of cigarette consumption and physical activity on respondent with positive perception, the difference can be assumed that positive perception provides a protective behavior was statistically significant. While different results were found in healthy diet comsumption pattern of respondent. There was no statistically different between positive perception and intention to consumpt healthy diet was reported. Therefore, it is suggested that an increase in the positive perception of healthy living will be accompanied by an increase in protective behavior toward non communicable disease. Conclusion: These findings suggest that positive perception on healthty life style such as non smoking and physical activity considered to have an important role in improving patients intetions to adopt healthy life style but not on healthy diet
