The use of both commercial and autologous fibrin glue in ophthalmology is still limited. Several researches about the effectiveness of commercial fibrin glue in pterygium surgery has been carried out. Previous studies found that commercial fibrin glue is a safe and effective method for attaching conjunctival autografts and results in shorter operating times and less postoperative discomfort compared to sutures method. While studies on autologous fibrin glue, which derived from patients own blood, showed that duration of surgery can be shortened and better conjunctival attachment stability can be achieved compare to suture method. There are, however, still very few studies comparing commercial and autologous fibrin glue for pterygium surgery. The objective of this study was comparing the surgical duration, degree of conjunctival autograft hyperemia and postoperative subjective symptoms (pain, watering, itchy and foreign body sensation) following the use of autologous and commercial fibrin glue for attaching conjuctival autograft after pterygium surgery. A clinical trial was carried out in 63 patients with primary pterygium grade 2. In 32 eyes, the transplant was attached to the sclera with a fibrin tissue adhesive (Beriplast-P) and 31 eyes with autologous fibrin glue. Follow up and standardised digital photographs were taken at 2 days, 1 week, 2 weeks and 4 weeks postoperatively to assess the degree of hyperemia. Subjective symptoms (pain, watering, itchy and foreign body sensation) were assessed using visual analogue scale. Surgical duration of autologous fibrin glue group was longer and statistically significant different between both groups (p<0.001). No significant difference was found in degree of postoperative hyperemia between two groups (p>0.05) in all follow ups. There was no significant difference of VAS score of pain at day 28 (p=0.062), and significant different at 3 follow ups before (p<0.05). There were no significant differences of VAS score of itchy at day 7 until day 28 (p>0.05). The foreign body sensation VAS score was not significant different at day 14 and 28 (p>0.05) and significant different at day 2 and 7 (p<0.05). The tearing VAS score were statistically significant different between both groups at all follow ups (p<0.05) We concluded that there was no significant difference in degree of hyperemia between commercial and autologous fibrin glue group.
