Vocal tract resonance characteristics are reflected by formant frequencies which are determined by the size and shape of the vocal tract. Tonsillectomy may cause changes in the structure of the oral cavity that serves as a resonator in the speech formation process which is seen from one of the voice analysis parameters i.e. formant frequencies. This study aimed to determine the effect of tonsillectomy with or without adenoidectomy on formant sound frequency. The study design was a pre-test and post-test only design experimental (before and after). Study subjects who meet the inclusion and exclusion criteria in the ENT department of dr. Sardjito General Hospital Yogyakarta, ENT department of dr. Soeradji Tirtonegoro General Hospital Klaten, Central Java and ENT Department of Saras Husada General HospitalPurworejo-Central Java were had a sound analysis by recording vowel / a /, / e /, / i /, / o /, and / u / in 1 day before and 4 weeks after tonsillectomy with or without adenoidectomy using Praat 1.5.05 software. The time period for this research between August until all the sampling data are fullfilled. This study uses a paired T test analysis for the numerical parametric variables comparative test. The result shows that the sample consisted of 24 women samples (60%) and 16 men samples (40%). The youngest age is 5 years old and the oldest was 42 years old. It was obtained the highest frequency in the underweight group were 19 samples (47.5%) and the least frequency in the overweight group were 6 samples (6%). The most tonsil size are T3-T3 which are 25 samples (62.5%) and the lowest is T3-T4/T4-T3 which are 6 samples (15%). Tonsillectomy operations types on all samples are 40 samples (100%). 18 samples (45%) with Guillotine method and 22 samples (55%) with dissection method. A significant mean increase occurs in almost all F1 vowel (p <0.05) except for / i / (p> 0,05). A significant mean decrease occurs in F2 / e / and F3 / u / (p> 0,05). Based on these results it can be concluded that tonsillectomy affects the F1 mean difference almost in all vowel except the vowels / i / and F2 & F3 largely unaffected.
