Summary:Pain consists of 2 types namely nociceptive and neuropathic pain. Neuropathic pain is a sensory and emotional experiences that has a significant impact on a patient's quality of life. Antidepressant drugs have been shown to be beneficial in diabetic neuropathic where the best effect is achieved by tricyclic antidepressant (TCA). Gabapentin as an antiepileptic drug has an analgesic effect. Herniated nucleus pulposus (HNP) is one of peripheral neuropathic pain. This study aimed to compare the therapeutic effect and quality of life of gabapentin and amitriptyline for the treatment of pain in HNP. The method was used a quasi experimental with consequtive sampling. This study included 30 patients who treated with gabapentin 100 mg two times a day and 26 patients who treated with amitriptyline12.5 mg daily. The therapeutic effect was assessed using Visual Analogue Scale (VAS) every 2 weeks for 1 month while quality of life was measured using Brief Pain Inventory at week-0 and week-4. This study was conducted in Neurology Department of Jogja Hospitalon April-July 2013. The comparation of therapeutic effect was analysed using Mann Whitney test, whereas the comparation of quality of life was analysed with independen sample t test. The results showed that there was a decline in Visual Analogue Scale (VAS) at week-4 in gabapentin and amitriptyline groups, decreasing for 3,700±0,349 and 3,500±0,343 point respectively, although there was no statistical difference (p value = 0.704). There was an increase in Brief Pain Inventory at 1,876±0,186 for patiens in gabapentin group and 1,868±0,222 in amitriptyline group, but there was no statistical difference in week-4 (p value = 0.977). To sum up, the therapeutic effect and quality of life between gabapentin and amitriptyline in the treatment of neuropathic pain did not have statistical difference as pain reliever in patients with herniated nucleus pulposus.
