Background: hospital health care facilities that are absolutely required by all levels of society in an effort to improve health status, both individuals and society as a whole. therefore hospitals should improve the quality of its services, including the improvement of the quality of medical records. The purpose of the medical record is to uphold the orderly administration in the context of efforts to improve health services in the hospital. Sheet medical resume is a summary or the core of the entire record of the treatment or management of medical records of the patients in the hospital, and a very important role in ensuring the quality of hospital medical implementation, especially in terms of completeness of medical record documentation. Based on early studies in hospital. Eye \"Dr. Yap\" the researchers found an incomplete medical resume sheet, while the sheet is a summary of the entire period of care and treatment of patients. Objective: To Knowing the percentage of completeness of data identification, important reports, and authentication resume sheet, to determine the factors incompleteness and efforts to improve the completeness sheet resume on file in the inpatient medical record Eye Hospital \"Dr. Yap \"Yogyakarta. Methods: type of study is a descriptive study with a qualitative approach to data collection using the technique of study documentation, interview and observation.
