Summary:Pneumonia disease kills more childrens under 5 years old than any other disease in every country troughout the world. Estimated nearly one-fifth of children's cause of death worldwide, approximately 2 million childrens under 5 years old die every year from pneumonia, mostly in Africa and Asia. From the children with pneumonia reported case data in Indonesia by 2011, about 168.019 infant under 1 years old cases pneumonia with 10.770 infant under 1 years old suffer from severe pneumonia, and 312.014 toddler from 1-4 years old cases suffer from pneumonia with 10.797 among them suffer from severe pneumonia. Pneumonia in the DR. R. Soetrasno Rembang Hospital listed in top 10 cause of hospitalizations disease. Based on data of DR. R. Soetrasno Rembang Hospital's medical records from 2012, two hundred and seventy-seven patients with pneumonia hospitalized. Two hundred and thirteen of them (56,5%) are children under 14 years old. The use of antibiotics evaluation in children with pneumonia at DR. R. Soetrasno Rembang has never been performed. This study was an analitical cross sectional study with retrospective data based on pediatrics medical record of patients with pneumonia whom admitted to the pediatrics wards in DR. R. Soetrasno Rembang Hospital from January 2012 to October 2013, and were assesed its rationality using Gyssen method and assesed its clinical outcomes results after using antibiotics for 3 days. The results showed that the use of antibiotics therapy given for the first time to patients with pneumonia in DR. R. Soetrasno Rembang Hospital pediatrics wards has been evaluated using Gyssen methods, 49,7% were rational and 50,3% were irrational. Rational use of antibiotics gives better clinical outcomes significantly (temperature, respiratory rate, retracti ons, leukocyte number, and segmental neutrophyl counts) than irrational use of antibiotics ( p < 0,05).
