Summary:Stroke is one of major world health problem that cause mental and physical disability in adult and elderly. Apolipoproteins (Apo) are the protein component of lipoprotein. ApoB is the major Apo in chylomicrons, Very Low Density Lipoprotein (VLDL), Intermediate Density Lipoprotein (IDL) dan Low Density Lipoprotein (LDL). ApoA-I is the major Apo in HDL. The apo B/apo A-I ratio represents the balance between atherogenic particles, which are rich in apo B, and apo A-I riched-antiatherogenic particles,. The aim of this study is to determine the role of Apo B/Apo A ratio elevation (> 0,9 in men dan > 0,8 in women) as a risk factor of ischemic stroke in Yogyakarta. This was an observational analytic study with case-control method. Cases of the study were ischemic stroke patients taken from Stroke Unit of Dr. Sardjito Hospital. Control subjects are patients in Neurology Ward Dr Sardjito Hospital with diagnosis other than stroke. Controls were matched with cases for age and gender. Univariate analysis of the risk factors for ischemic stroke was performed by calculating odds ratios (OR) with 95% CI, p <0.05. Multivariate analysis was performed with logistic regression test. The results showed history of diabetes mellitus, hypertension, heart disease, total cholesterol, HDL, LDL and Apo B/Apo A1 ratio were statistically significant in univariate analysis as risk factors of ischemic stroke. But in multivariate analysis, only hypertension (OR 3.531, 95% CI, 1.138 to 10.954, p = 0.029) and Apo B / Apo A1 ratio showed significant results (OR 4.113, 95% CI, 1.443 to 11.723, p = 0.017). The ratio of Apo B / Apo A1 was better than the conventional lipid profile and acted as an independent risk factor for ischemic stroke. Keyword : ischemic stroke, risk factor, Apo A1, Apo B
