Summary:Background: In developing countries, approximately 30-70% of workers are in the informal sector where the majority are economically weak. Data from BPS of Riau Islands Province, informal sector workers in February 2013 was 280.951 people (31.62 %). One of Problem still faced by informal sector workers today is the difficulty of getting health care. Puskesmas as the first level of health care facilities in Indonesia need to provide a solution to the health and safety needs of the informal sector. Objectives: This study aims to determine the role of puskesmas in development of occupational health program at Tanjungpinang, which include the implementation of the program, support factors, constraint factors, and the need of occupational health and safety workers in informal sectors, also the need of national health insurance. Methods: This is a descriptive qualitative research with case study approach. Location of this study at Puskesmas Sei. Jang in Tanjungpinang. The research was conducted in February until May 2014. The subjects of the study were leader of puskesmas, coordinator of occupational health in informal sector, and traditional fisherman. Data collection was conducted by interview, focus group discussion, and observation. Results: Implementation of occupational health program in Puskesmas Sei. Jang which includes coaching, service, and sustainability. Supporting and inhibiting factors in implementation of occupational health in informal sector are influenced by internal and external factors, such as the human resources, budget, commitment, and community conditions. Traditional fisherman needs occupational health and safety program because lack of knowledge and awareness for the use of personal protective equipment while working. Therefore, the traditional fisherman need the national health insurance to be able get health care service. Most of traditional fisherman do not understand about national health insurance and refuse to pay independently, because of their income is not settled Conclusion: Overall, occupational health and safety program in informal sector has been running well. The role of puskesmas in development of occupational health and safety program in informal sector is still running, although there are several inhibiting factors that need to be addressed.
