Summary:The use of the hospital formulary that is not optimal will decrease the quality of the health service. The formulary of the Local Hospital of Yogyakarta has not been revised in seven years. The use of the formulary in the prescription was also very low. The objective of this research is to find out the increase in use of the revised formulary in prescriptions and to see the effect of Monitoring and Evaluation (Mon-Ev) on prescriptions. The research method used was quasi experimental using the pre and post test without control.The subjects of the research are all physicians and the prescription analysis unit. The research involved were revising the formulary and Mon-Ev intervention.The observation undertaken involved measuring the increase in the percentage of the use of the revised formulary in prescriptions. In the pre and post MonEv intervention, prescription indicators were observed. The pre and post Mon-Ev data were analyzed statistically using a paired t-test. The result of the research after the revision of the formulary was that the use of the formulary in prescriptions increased. The result of Mon-Ev intervention was the use of the formulary in prescriptions in the pre and the first post Mon-Ev (P>0.05), in the pre and the second post Mon-Ev (p>0.05) and in the first post Mon-Ev to the second post Mon-Ev (p
