Background: Concerning the weakness of the current structure of Dinas Kesehatan Kabupaten Rembang, the intention of improving efficiency and performance of health services, and the opportunity of decentralization policy, restructuring was conducted at Dinas Kesehatan Kabupaten Rembang. Method: A case study is conducted to review the process of restructuring and how the facilitation process during a five-day workshop help the restructuring process to attain the purpose of restructuring. The workshop is facilitated by facilitators from Pusat Manajemen Pelayanan Kesehatan (PMPK) Universitas Gadjah Mada and Balai Pelatihan Teknis Profesi Kesehatan (BPTPK) Gombong. The workshop process and the results are documented, reviewed, and reported as a basis for advocacy, socialization, and further discussion with local government and legislature. Result and conclusion: A five-day workshop is the core process of restructuring. The result of the restructuring workshop is a document consisting the background and the purpose of restructuring, the strategic analysis of restructuring, the draft of new structure including the roles and functions of Dinas Kesehatan Kabupaten, Cabang Dinas that is later called as Unit Pelaksana Teknis Daerah Pusat Pencegahan Penyakit dan Promosi Kesehatan (UPT-P4K), and Pusat Kesehatan Desa (Puskesdes). Active involvement and commitment of the workshop participants, the stakeholders of Dinas Kesehatan Kabupaten Rembang is the key for success of the workshop process. The restructuring process follows the steps of organizational redesigning. Reorganization, devolution, and self provision through community empowerment are the strategy for Dinas Kesehatan Kabupaten Rembang restructuring.
