Summary:Background: The government is currently emphasizing health insurance for poor community through community health insurance program as part of comprehensive health development. To ensure access of poor community to health service, district government of Ngada implements the program of poor community health maintenance through local health insurance program (Jamkesda). The target of the program is poor community not covered in the central government scheme and other health insurance mechanisms. The quantity of poor community at District of Ngada covered by this local scheme are as many as 8000 people. With the availability of the insurance poor community can utilize health service as necessary. However, in reality the utilization of health service by poor community is still relatively low. Objective: To identify factors associated with the utilization of health service by poor community at District of Ngada. Method: This analytical study used cross sectional design and quantitative as well as qualitative methods. The study was undertaken at 4 health centers (2 at urban area and 2 at rural area. The analysis unit were poor communities at the health centers of Kota, Waepana, Natarandang and Riung. Subject of the study consisted of poor communities. Samples were poor community that had been purposively selected. Data were obtained from questionnaire and interview guide. Analysis of quantitative data used bivariate with contingensi coefisient statistical test, multivariate with logistic regression. Result: Out of 344 respondents, 60.5% utilized free health service at District of Ngada. Factors significantly associated (p<0.05) with the utilization of health service by poor community were perception about the benefit of Jamkesda card for poor community, perception about bureaucracy administration, perception about extra cost, perception about service and access. Factor most dominantly associated with behavior in the utilization of health service is perception about extra cost (Wald= 47.707). The reason why poor community did not utilize health service were drug cost and the amount of transportation cost that had to be paid to reach health facilities. Conclusion: Half of poor community participants of Jamkesda at District of Ngada had not utilize free health service provided by the government. Factors most dominantly associated with the utilization of health service were perception about extra expenditure in the form drug cost.
