Summary: | Background : Diabetes mellitus (DM) is a metabolic syndrome characterized by
hyperglycemia and glucose intolerance, due to insulin resistance and insulin
deficiency, or both. Retinopathy diabetics (RD) is a diabetes mellitus
complication due to retinal abnormality, causing vision reduction, even blindness.
Previous studies showed association between RD and aldose reductase C-106T
gene polymorphism. This genetic polymorphism increases the level of sorbitol
inside erythrocyte and pericyte in the retinal membrane leads to weakness of
retinal capillary vessel and microaneurism.
The aim of study : To know the presence of ALR C-106T gene polymorphism
and frequency distribution among Javanese ethnic group in Dr. Sardjito Hospital
Yogyakarta. To analyze the different of Erythrocytes osmotic fragility (EOF)
among ALR genotypes and between DM type 2 with RD and without RD. To
analyze ALR genetic polymorphism as a risk factor of DM type 2 with RD.
Methods : Using case control study involved 40 patients with RD as case and 40
patients without RD as control groups. The C-106T ALR gene polymorphism
was determined by PCR�RFLP method. EOF was analyzed by spectro. Genotype
and allele distribution was analyzed using x

independent t-test and Mann-Whitney with p<0.05 considered as significantly
different.
Result : The genotype distribution among patients with RD were CC 33 (82.5%),
CT 7 (17.5%) and among patients without RD were CC 27 (67.5%), CT 13
(32.5%). The genotype and allele distribution were not significantly different
between two group (p=0.121 for genotype, p=0.151 for allele). Odds Ratio of
genotype is 2.270 while allele is 2.023. EOF of CC genotype was higher that of
CT genotype (p=0.047).
Conclusion : The data showed not significant difference between CC and CT
genotype distribution among type 2 DM patient with and without RD. EOF of CC
genotype was higher that of CT genotype. C-106T gene polymorphism is a risk
factor of retinopathy diabetics in type 2 DM. |
