As an educational institution, schools have a strategic role and position in the efforts of health promotion about the dangers of smoking. One of the efforts undertaken to protect the school from the illness caused by smoking is the provision of health promotion using a medium of picture cards that can be delivered by a method of learning while playing, which is in the form of a quartet card game that contains pictures about the dangers of smoking. Objective: To determine the effect of the use of picture cards as a medium to increase knowledge on health promotion and attitudes of the dangers of smoking among students of Karangmangu Elementary School of Banyumas District. Methods: This was a quasi-experimental study with a before and after intervention study design by using a referent group. Samples were taken in a purposive sampling method as many as 40 students in each group. The treatment of health promotion was given to the experimental group by using picture cards as the medium of health promotion, whereas in the referent group they were only given education. The data were collected in both groups with a time interval before and after treatment of one month. The data obtained were processed using statistical tests of independent sample t-test and logistic regression with p < 0.05. Results: The increasing knowledge and attitudes against the dangers of smoking in the experimental group were better than those in the referent group. There were significant differences on improving the knowledge and attitudes between the respondents of both groups.
