Management and interaction among ICU caregivers needs to explore to find out how actual utilization of ICU in RSUD Tamiang Layang. Methods: This study used a qualitative descriptive case study design, with using a holistic single case design. The analysis unit is RSUD Tamiang Layang. Depth interviews were conducted on 13 respondents consisting of the Director of RSUD Tamiang Layang, Head of Medical Services, Head of Allied Medical Services, and 8 ICU�s nurses. Sources of data obtained from observation, depth interviews and document study. Results: The management of staff in the ICU hospitals have started using open system management. Interaction of ICU�s caregivers which are the form of leadership, communication, coordination, and conflict management conducted in an open and flexible. ICU utilization of RSUD Tamiang Layang is a strategy of the hospital management to meet market demand which is an intensive services for emergency patients. It also proves that the utilization of type D hospital can perform the same function as type C hospital because of market opportunities in the region. Conclusion: Necessary education and training for hospital ICU staff to improve the performance of the ICU. In addition, because management has begun to open system and market opportunities that are still extensive, cooperation with other sector need to be done to meet the shortage of resources.
