Phase one comprised of a cross-sectional study among patients with T2DM who had undergone the treatment in two public hospitals in Klang Valley, Malaysia. Medication Compliance Questionnaire was used and, bivariate and multivariate analyses were conducted. Phase two involved in-depth interviews with 12 non-adherence patients and were analysed using constant comparative. Results: Nearly half of the patients failed to adhere to the medication (46.4%). Malays, single/widow or divorced, with poor Haemoglobin A1c were most likely to not adhere. Five factors for medication non-adherence including perceived benefit of complementary and alternative medicine, attitude towards drawback of western medication, poor healthcare providers and patients’ relationship, undesirable emotional response towards medication intake, and restraint in daily routine. Conclusions: There are many reasons for patients’ non-adherence to their anti-diabetes medication.
