This Web based Information System will be al lot of help to De La Salle University-Dasmarinas Clinic. It can improve the circulation, manipulation and distribution of data records within the three outpost of the clinic. It can also be a great solution to the most common problems occuring in the clinic. The system provides a portal where students, staffs and faculty members can update medical information and request physical exam online. The system will also allow nurses and doctors to view and retrieve records easily. Replenishment of medicine will be monitored automatically and accurately. Each student, faculty member and staff will be given an account to access the website.The personnel in charge on the clinic will also have an account which cannot be accessed by unauthorized people and will be use for administration purposes. The paper works, forms and logbooks will be minimized by the system. The reports will be generated by the system automatically. Keeping of data recoards will be saved in a secured database. Loss of important records will be avoided using the system.
