In healthcare, more often than not, we are still asked to fill in forms when we arrive for registration purposes. The pre-consultation is when a patient visits a clinic, typical questions such as past medical history, current condition, and any major illness that needs to be addressed are usually asked verbally or through a questionnaire given by the clinic. The receptionist then processes the information and manually keys into the system, or even in some situations, the patient information are written manually on a physical card. The purpose of this project is to help in advancing the way we collect such information, by developing an iOS mobile application that can gather such information instead of paper forms, and to store digitally. This process could have been automated or advanced with technology so that repetition for every patient that visits the clinic could be avoided, which could be very efficient in the long run.
