Visual and Metallographic Examinations were conducted on the cracks with photo-documentary proof. The main mechanism of failure to the engine was corrosion fatigue, which appeared to be due to the corrosiveness of the water in the cooling system, in combination with the high operating temperature and vibration on the engine. Several recommendations on improvement to the current maintenance practices were suggested for consideration.
