This final year project aims to build an Android phone application which can help chronic disease patients manage their health conditions more conveniently, especially for diabetics. This is because diabetes needs more monitoring than other chronic diseases. Patients normally need to record their blood sugar level at least twice a day. Moreover there are many factors to affect the blood sugar level. This application provides services in record creation, generating statistics, charts and emailing reports to doctors. These are the basic functions for diabetics to track their health conditions. They can monitor the situation by observing different types of charts and can also seek advices from doctors by emailing their reports.
