Summary:Stroke is one of the top ten causes of death and disability in Singapore and many stroke patients are heavily dependent on rehabilitation services. There is an ever increasing need for setting up better health care facilities through the means of using telecommunication technologies. Thus, this Final Year Project (FYP) report will be covering the motivation and objectives of setting up tele-rehabilitation for stroke patients using the Personal Health Information Management (PHIM) system. A literature review on telerehabilitation and its technologies used will be provided in this report. In addition, the software requirement for setting up tele-rehabilitation will be further explained in this report. An analysis on the plans executed for this project and the necessary implementation will also be given in this report. The tele-rehabilitation software performance and discussions on the performance will be included in this report. Last but not least, a conclusion of the lessons learnt from this project will be included in this report. The recommendations for future development will be provided in this report, as a contribution to the medical community and experts for their future research.
