Femoral neck fractures are a common form of injury amongst the elderly in which cannulated screws have been used as the choice for fixation. Previous studies have been performed to gauge the effectiveness of various screw designs however, they have correlated pull-out force with the effectiveness of fixation. In this study, the objective was to determine which type of screw offered the best fixation based on displacement, load and stiffness characteristics. Two grades of synthetic bones with simulated femoral neck fractures were fixated with single inferior screw triangle configurations using either 6.5 mm non-cannulated, 6.5 mm cannulated or 7.3 mm cannulated orthopedic screws. Using the Instron 5566 Universal Testing Machine, compressive tests were conducted with progressive loading to failure at 5 mm/min with the specimens oriented in the stance gait. Twenty-seven specimens were tested. Differences in displacement, load to failure and stiffness were analyzed using the Student's t-test. It was found that in Grade A synthetic bone, 6.5 mm non-cannulated screws offered significantly greater load to failure (p < 0.05) when compared to 6.5 mm cannulated screws. The results suggested that for orthopedic screws of the same diameter, non-cannulated screws offered better fixation in terms of load to failure.
