Summary:With the rapid growth of the elderly population and a rise in younger aged people being diagnosed with dementia, dementia sufferers are projected to increase exponentially in the next 30 years. Also, treating and caring for dementia patients have cost the world more than US$604 billion per year. The absence of a cure or remedy for dementia has encouraged researchers over the world to seek a solution and stress the importance of dementia care in improving a patient’s life. The aim of the project in this report is to create a person-centred care system, and to explore the uses of this system in achieving the overall objective of improving dementia patients’ lives by increasing the efficiency of dementia care. The person-centred care system in this project is named Pear, and Pear is being created for a particular dementia day care centre operated by the Alzheimer’s disease Association based in Singapore. Pear has a central repository which stores all information of patients in the care centre. Pear has three entities: web application, android application and android games. The Android application is designed to allow quick access to patient’s data and brings convenience to the health care providers. This application allows easy managing of patient’s information which includes patient’s photo album and background information. Also, the system has an automated personalized patient’s schedule generator which generates schedule based on each patient’s past and present records. Furthermore, there is an in-app notification function which acts as an approval/rejection platform for change of patient’s data. Lastly, the application allows the health care providers to view the patient’s schedule for the day. Pear was carefully designed using the waterfall model and follows best practices to ensure maintainability, flexibility and user experience as its priority. Pear’s objective is to improve the efficiency of Person-Centred Care operations and to leverage on technology all in hopes to improve a dementia patient’s quality of life and family experience. I believe that this application can help to improve the productivity in terms of time hours in a person-centred care environment. This application aims to reduce the health care providers’ burden to perform person-centred care.
