Singapore is predicted to have an increase in the number of dementia patients. Studies have shown that having a healthy lifestyle, exercising regularly and watching your diet can reduce the risk or prevent dementia. With an algorithm, all patients in the person-centred care will have activities automatically scheduled for them. To plan the most suitable activities, a set of information is gathered from the patient, their family members, other healthcare providers and doctors to help the patient maintain a healthy lifestyle., An automated testing tool will ensure the quality and validity of the generated patient’s schedule. All information in the person-centred care will be stored on a centralized database to give easy access to doctors, supervisors, caregivers, game therapist and guardians to manage patient’s information. The centralized database is accessible mainly by two teams, the front-end team and the backend system team. The web interface is made available to doctors, system administrators, game therapist, and guardian. Doctors shall be able to manage and monitor their patient’s information and prescribe games to the patient. System Administrators shall be able to manage user accounts and games, view user’s activity log and manually generate patient’s schedule. The game therapist shall be able to manage games and perform game analysis on patients. Guardian shall be able to provide social history information on behalf of a patient. Social history information is patient’s personal information. This project is a continuation from the author’s predecessor. The author’s predecessor has developed a Version 1 system for the Person-centred care. The author will enhance and further develop the Version 1 system, to produce a Version 2 system. The person-centred care system aims to provide the person-centred care an efficient way to monitor and manage their patient using current technology, providing them with better health care support.
