Abstract: Increased prevalence of chronic illness is associated with the aging of the world population and its impact on behavioural changes of the individual, family, community, and health care providers. Selfmanagement concepts demonstrate the best evidence of health outcomes. Current health care systems are not sustainable, but many forms of self-management concepts are widely developing as a part of the solution based on a client-centered intervention and support of health care providers in Thailand.
