Data were initially gathered using a questionnaire-based pilot survey in Google form to identify dental practitioners who had used silver diamine fluoride. Then, in-depth interviews with eight dentists, six professors , five lecturers from Sirindhorn College of Public Health, two silver diamine fluoride experts, and four dental policy makers were conducted to understand the enabling conditions and impediments to the use of silver diamine fluoride adapted from “The concept of access” by Penchansky and Thomas 1981. The findings revealed that the enabling factors are the acceptance of caries arrested efficacy, simple procedures, less material and time, a small volume while applying, and prior experience with silver diamine fluoride in children. Teaching the topic of silver diamine fluoride in the undergraduate dentistry curriculum or other courses results in dental practitioners' perception of silver diamine fluoride. Furthermore, Covid – 19 pandemics caused the inability to do aerosolizing treatments, which impelled dental practitioners curious about employing silver diamine fluoride. Black staining following silver diamine fluoride application, flavor, and acerbity, as well as the lack of a silver diamine fluoride code in the systems, are barriers to use. At present, silver diamine fluoride is not listed in the benefit packages of Thailand’s Universal Health Coverage and has no policy explicitly encouraging its usage resulting in an ambiguous problem for entering the code after treatment. Possibly due to dental policy makers’ opinions, though that silver diamine fluoride is not a treatment option for everyone, we have to choose cases appropriately, and also, there is no substantial academic evidence-based information on the use of silver diamine fluoride among the elderly in Thailand. การศึกษานี้มีวัตถุประสงค์เพื่ออธิบายปัจจัยที่สนับสนุนหรือเป็นอุปสรรคต่อการใช้ซิลเวอร์ไดเอมีนฟลูออไรด์ในผู้สูงอายุในกลุ่มทันตบุคลากรที่ปฏิบัติงานในภาคตะวันออกเฉียงเหนือ โดยมีการสำรวจเบื้องต้นผ่านแบบสอบถามออนไลน์เพื่อค้นหากลุ่มเป้าหมายที่มีประสบการณ์ในการใช้ซิลเวอร์ไดเอมีนฟลูออไรด์ จากนั้นทำการสัมภาษณ์เชิงลึกในทันตบุคลากร 8 คน อาจารย์มหาวิทยาลัย 6 คน อาจารย์วิทยาลัยการสาธารณสุขสิรินธร 5 คน ผู้เชี่ยวชาญด้านการใช้ซิลเวอร์ไดเอมีนฟลูออไรด์ 2 คน และผู้ที่มีส่วนเกี่ยวข้องกับการออกนโยบาย 4 คนเพื่ออธิบายปัจจัยที่สนับสนุนหรือเป็นอุปสรรคต่อการใช้ซิลเวอร์ไดเอมีนฟลูออไรด์ ตามแนวคิดที่ประยุกต์มาจากแนวคิดการเข้าถึงบริการสุขภาพของ Penchansky and Thomas 1981 ผลการศึกษาพบว่าปัจจัยที่สำคัญที่สนับสนุน ได้แก่ การยอมรับประสิทธิภาพของซิลเวอร์ไดเอมีนฟลูออไรด์ในการหยุดยั้งฟันผุ มีขั้นตอนการใช้ที่ง่าย อุปกรณ์น้อย ใช้เวลาไม่นาน ใช้ปริมาณต่อครั้งน้อย และการมีประสบการณ์ใช้ซิลเวอร์ไดเอมีนฟลูออไรด์ในกลุ่มเด็กมาก่อน การเรียนการสอนเรื่องซิลเวอร์ไดเอมีนฟลูออไรด์ทั้งในหลักสูตรของคณะทันตแพทยศาสตร์หรือการอบรมนอกหลักสูตรส่งผลต่อการรู้จักซิลเวอร์ไดเอมีนฟลูออไรด์ของทันตบุคลากร อีกทั้งการระบาดของโรคโควิด – 19 ที่ทำให้ไม่สามารถทำงานฟุ้งกระจายได้เป็นส่วนผลักดันให้ทันตบุคลากรเกิดความสนใจ ปัจจัยที่เป็นอุปสรรค ได้แก่ การติดสีดำหลังทาซิลเวอร์ไดเอมีนฟลูออไรด์ รสชาติและความแสบขณะทา รวมถึงการไม่มีรหัสบันทึกการใช้ซิลเวอร์ไดเอมีนฟลูออไรด์ในระบบ ปัจจุบันยังไม่มีนโยบายที่ส่งเสริมการใช้ซิลเวอร์ไดเอมีนฟลูออไรด์ในผู้สูงอายุโดยตรง เนื่องจากผู้มีส่วนเกี่ยวข้องกับการออกนโยบายให้ความเห็นว่าซิลเวอร์ไดเอมีนฟลูออไรด์ไม่ใช่การรักษาที่สามารถให้ได้ในประชาชนทุกคนจำเป็นต้องเลือกเคสให้เหมาะสม รวมถึงหลักฐานทางวิชาการและข้อมูลการใช้ซิลเวอร์ไดเอมีนฟลูออไรด์ในกลุ่มผู้สูงอายุของประเทศไทยยังมีไม่เพียงพอ 2023-12-18T04:32:52Z 2023-12-18T04:32:52Z 2022 Thesis http://kb.psu.ac.th/psukb/handle/2016/19179 th Attribution-NonCommercial-NoDerivs 3.0 Thailand http://creativecommons.org/licenses/by-nc-nd/3.0/th/ application/pdf มหาวิทยาลัยสงขลานครินทร์ |
