This research aims to analyze and evaluate the implementation of OHSMS by construction companies (contractors) who are handling the new building construction in ITB Ganesha using Root Cause Analysis (RCA) and Failure Modes and Effects Analysis (FMEA) Methods, the data sources are primarily from observation results in site, documentation, work accidents data recorded when the research, questionnaires, and interviews, meanwhile other data obtained/secondary are internal organization procedures/policies associated with OHS Management System guidance for each contractors, government regulations and legislations, quality standards, recent scientific references, and work accidents data of contractors. The results of RCA Method for work accidents show that there is most fatality work accident that is fall from 20 meters height when worker executed steel erection for ITDCTM-ME Project with risk index 15 and have extreme category, and construction dirt fall over on the worker’s head while the worker is resting upon the borepile for ITB (III) Development Project with risk index 8 and have high category, meanwhile the results of FMEA Method show that ITDCTM-ME Project has the most widely high category of risk potential, 23 works for civil and structures (95 %) and 28 works for architectural (93 %). This research expressing the conclusion that detectability is the most influential value for Risk Priority Number (RPN). text
