Summary:RS Ananda Putri is a new general hospital established since 19 September 2016 under Yayasan Winda Nauli Medan. Since October 2017 there has been a significant decrease in the number of patients. The author conducted interviews with the Director of Ananda Putri Hospital and made observations to determine the cause of the problem. Based on the interview result and field observations, the authors found the existence of human resource issues where employees do not provide excellent service and SOP not run properly. Moreover the absence of BPJS facilities make patients reluctant to seek treatment at the hospital. To overcome these problems the hospital plans to add BPJS in RS Ananda Putri, but the hospital is required to follow the accreditation first where the accreditation aims to assess the performance of the hospital. Therefore, the authors plan to proposed a performance appraisal system for RS Ananda Putri. <br /> <br /> <br /> The performance appraisal system used in RS Ananda Putri is a form that contains indicators, the identity of the assessed employees, the time period and the appraiser for assess the employee performance to be performed more precisely and has an assessment standard to curb subjectivity since current performance appraisals are conducted only on the director's observations without standard pointed. In preparing the performance appraisal in this research, several approaches are conducted in collecting data. The research method used by the authors in this study using qualitative methods which is conducting interviews to employees and directors of the hospital and direct observation to see job descriptions of employees, especially in the emergency unit which is the focus of the author because the ER unit is the entrance to the patient who presented the overall hospital services. <br /> <br /> <br /> The results of this performance appraisal system is expected to be a guide for the hospital to measure the performance of its employees clearly and objectively. The employee performance appraisal using Key Performance Indicator (KPI) is compiled using SMART characteristics and each component's weight is calculated using AHP which is expected to result in a more fair appraisal system for all employees. <br />
