The example failure in oil and gas industry is a failure that initiated by a small leak in the pressure boundary of the equipment that caused by some degradation mechanism. Therefore, it is needed to do the risk assessment associated with the equipment in attempt to know the level of risk of the associated equipment. <br /> <br /> This final project is intended to develop a calculation program of risk value based on API 581 2016 edition standard an then also foccused on risk assessment on the case of production separator. <br /> <br /> The probability of failure result of the calculation with considering the operation and environmental condition is 9,746 x 10-4 failure/year and 427,77 m2 for flammable consequence at component damage category and dan 926,52 m2 at personnel injury category and 5.969,9 m2 for toxic consequence. The position of this condition is on 3C and 3D at risk matrix where this position categorized as a medium and medium high risk. <br /> <br /> This final project also carried out about inspection planning to reduce the risk value in 10 years period and determine the how much impact the risk value reduction due to inspection and obtained up to 90% reduction of the initial risk value.<p align="justify"> text |
